Job summary

NorthMed Health is partnering with a well-established physical therapy practice on the North Shore of Massachusetts to hire a full-time Physical Therapist (PT) to provide outpatient treatment in patients’ homes, offering variety, flexibility, and meaningful one-on-one care.

This home-based role suits clinicians who value autonomy, build strong patient relationships, and want a balance between clinic and community care with a manageable caseload of 30–32 visits per week.

Qualifications

• Licensed Physical Therapist (PT) or license-eligible in Massachusetts.
• Outpatient or home-based experience preferred; new grads considered.
• Vestibular experience is preferred, but willing to train.

Responsibilities

• Provide outpatient physical therapy care in home-based settings.
• Perform comprehensive evaluations and develop individualized plans of care.
• Deliver evidence-based treatment focused on functional mobility and independence.
• Educate patients and caregivers on exercises and recovery strategies.
• Complete timely and accurate documentation.
• Collaborate with other therapists and leadership to ensure continuity of care.
