Job summary

Qualifications

• Bachelor's degree in OT from an accredited program.
• Active licensed OT in the practicing state and CPR/BLS certification.

Responsibilities

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs and develop personalized plans.
• Teach patients and families about home safety and adaptive equipment.
• Maintain accurate progress notes and adjust therapy programs as needed.
• Collaborate with physicians, nurses, and other home health team members.

Skills

Active registered OT license

CPR/BLS Certification

One-year OT experience

Driver's license

Education
