Job summary

CommonSpirit Health at Home seeks an experienced Occupational Therapist to provide in-home therapy in Leadville, Fairplay and Frisco on a PRN basis. The OT will evaluate patients, set goals, and design a plan to improve independence and quality of life.

The role involves coordinating with physicians and the home health team, educating families on safety and adaptive equipment, and maintaining accurate documentation to meet regulatory standards.

Qualifications

• Bachelor's degree from an accredited school approved by the American Occupational Therapy Association
• Active registered OT license in the state which you practice
• Must obtain and maintain current CPR/BLS Certification
• One-year Occupational Therapy experience in a clinical setting
• Current Drivers License
• Comply with federal, state and local laws and regulations

Responsibilities

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs. Develop and implement personalized treatment plans that may include exercises, adaptive techniques, and use of assistive devices to improve patients' functional abilities.
• Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.
• Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
• Monitor patients’ progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.
• Collaborate and communicate patient’s change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.
