Job summary

CommonSpirit Health at Home is seeking a skilled Occupational Therapist to join our home health team as a PRN caregiver. You will assess patients in their homes, set goals, and develop plans to promote independence and improve quality of life.

You will educate families on safety and adaptive equipment, document progress, coordinate with the care team, and adjust treatment as patients progress under regulatory standards.

Qualifications

  • Bachelor’s degree from an AOTA-approved program.
  • Active registered OT license in the state where you practice.
  • Current CPR/BLS Certification.
  • One year of Occupational Therapy experience in a clinical setting.
  • Current Driver's License.
  • Compliance with federal, state and local laws and regulations.

  • Educate patients and family members on home safety, adaptive equipment, and strategies for daily activities.
  • Maintain accurate progress notes, treatment plans, and modifications for compliance and regulatory standards.
  • Monitor progress and adjust treatment plans as needed using outcome-based measures.
  • Collaborate with physicians, nurses, and other home health team members to communicate changes in condition.

Job Summary and Responsibilities

This is a PRN Position

We are seeking a skilled and compassionate Occupational Therapist to join VNA Home Health team. The OT will provide skilled therapy services to patients in their home evaluating the patients abilities and needs, setting goals and developing a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems.

Responsibilities:
  • Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs. Develop and implement personalized treatment plans that may include exercises’, adaptive techniques, and use of assistive devices to improve patients functional abilities.
  • Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.
  • Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
  • Monitor patients progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.
  • Collaborate and communicate patient’s change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.
Job Requirements
Qualifications:
