Job summary

CommonSpirit Health at Home seeks a skilled Occupational Therapist to join our home health team. The OT will evaluate patients, set goals, and develop a plan to promote independence and improve quality of life in patients' homes.

Responsibilities include assessing functional abilities, teaching home safety, maintaining records, and collaborating with physicians and the broader home health team to ensure compliant, high-quality care.

Qualifications

• Bachelor's degree from an accredited school approved by the American Occupational Therapy Association.
• Active registered OT license in the state in which you practice.
• Current CPR/BLS Certification.
• One year of Occupational Therapy experience in a clinical setting.
• Current driver's license.
• Compliance with federal, state and local laws and regulations.

Responsibilities

• Assess patients' physical, cognitive, and psychosocial abilities to determine therapy needs and develop personalized treatment plans.
• Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities.
• Maintain accurate and timely records of patient progress and treatment plans for compliance and regulatory standards.
• Monitor patient progress and adjust treatment plans as necessary to meet changing needs.
• Collaborate and communicate patient condition changes with physicians, nurses, physical therapists, speech therapists, and other home health team members.

Education
