Qualifications

• Collaborate with reablement staff on interventions and goal planning.
• Triage reablement cases and determine priorities.
• Assess for and provide specialist equipment, including seating, and complete moving and handling assessments.

Responsibilities

• Conduct occupational therapy assessments of clients in their own home for standard equipment and minor adaptations.
• Work with reablement senior support workers to plan interventions and goals.
• Triaging of reablement cases to prioritize actions.
• Assess and supply specialist equipment, including seating; perform Moving and Handling assessments.
