Qualifications

• Current NP licensure and competency in primary care management.
• Strong clinical judgment and experience managing complex patient cases.
• Ability to create and apply standardized workflows and clinical standards.
• Effective communication, mentorship, and team-based collaboration skills.
• Comfort working in community-based, patient-at-home care environments.

Responsibilities

• Provide in-home assessments, diagnoses, and ongoing management for patients.
• Develop, implement, and adjust complex care plans to ensure patient-centered outcomes.
• Establish and standardize clinical workflows and care protocols for consistent quality.
• Coordinate with leadership and community partners to support market readiness and integration.
• Train, mentor, and guide local clinicians to build capability and ensure care consistency.
• Collaborate with care teams to maintain high-quality, evidence-based clinical practice.
