Reliant Medical Group, part of Optum, seeks a Nurse Practitioner or Physician Assistant to deliver in-home primary care to frail geriatric patients in the MetroWest region. You will own your panel and work with a supportive team, managing about 6–8 visits per day with autonomy and strong clinical backing.
Massachusetts licensure (or ability to obtain), plus National Board Certification and BLS are required. Travel reimbursement is provided along with a comprehensive benefits package.

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
