Job summary

Grace at Home in Indiana is seeking an Advance Practice Nurse Practitioner or Physician Assistant to provide Home-Based Primary Care in patients' homes or Assisted Living communities. You will manage a panel of patients, diagnose, treat, and coordinate care with MD oversight.

Responsibilities include in-home assessments, chronic disease management, medication reviews, preventive care, and caregiver education, with emphasis on HIPAA and travel up to 5 days per week as needed.

Qualifications

• Minimum 2+ years of clinical NP/PA experience in primary or home care.
• Unrestricted IN license as NP or PA.
• Nurse Practitioner certification from ANCC or AANP.
• Knowledge of chronic care management and medications.
• DEA license / Prescriptive Authority post-hire.
• Experience in Family/Internal Medicine, Gerontology, Palliative Care.
• BLS or ACLS certification.

Responsibilities

• Provide Home-Based Primary Care in patients' homes or assisted living communities.
• Manage a panel of patients with MD oversight.
• Perform comprehensive in-home assessments and chronic disease management.
• Review symptoms, perform examinations, and coordinate care with PCP.
• Educate patients on disease processes, medications, and adherence.
• Ensure HIPAA compliance and protect health information.
• Travel up to 5 days per week to patient homes as needed.
