Job summary

Grace at Home seeks a Nurse Practitioner or Physician Assistant to conduct in-home and telehealth visits, coordinating with a multidisciplinary team to deliver high-quality care. The role emphasizes value-based, home-centered care, chronic disease management, and patient education.

You will support Medicare/Medicaid programs, maintain accurate documentation, and collaborate across clinical teams in a fast-paced home-care setting.

Qualifications

• Master’s degree in nursing or related science field required.
• Active and unrestricted NP/PA license in state of hire.
• 2+ years direct patient care as NP/PA strongly preferred.
• Experience with Medicare/Medicaid preferred.
• Strong communication and analytic skills.

Responsibilities

• Deliver in-home and telehealth visits (AWVs, comprehensive assessments, follow-ups).
• Provide value-based, home-centered care focused on outcomes.
• Develop and implement individualized care plans with multidisciplinary teams.
• Participate in daily huddles and team meetings.
• Prescribe medications and therapies within scope of practice.
• Maintain accurate EMR documentation and ensure HIPAA compliance.
