Job summary

Ennoble Care is seeking an experienced Full-time Nurse Practitioner or Physician Assistant to deliver primary and palliative care to patients in the Volusia, FL region. Clinicians visit patients at home, applying medical techniques under physician supervision and managing chronic conditions.

The ideal candidate has 1-2 years in primary care with geriatrics; hospice experience is a plus. Flexible schedule with autonomy, strong support, and growth opportunities within a growing team.

Qualifications

• Board Certified, with a valid NP or PA certification in the State of Florida.
• 1-2 years of primary care experience with geriatric patients; hospice experience preferred but not required.

Responsibilities

• Provide high quality primary and palliative care to patients in the Volusia, FL region.
• Perform physical examinations and develop care plans for diverse patient needs.
• Communicate effectively with office staff and clinical operations across platforms.
• Attend quarterly medical staff meetings and participate in care coordination.
• Develop impressions, diagnoses and treatment plans; discuss options with patients and families.
