Rehab Without Walls Neuro Rehabilitation is expanding its Home and Community Neuro Rehabilitation Program in the San Francisco Bay Area, seeking compassionate Physical Therapists to support clients in their homes and communities. You will assess, treat, and empower individuals with brain injuries, stroke, and other neurological conditions to regain independence and meaningful daily activities.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program
• Current, unrestricted PT license in the practicing state
• BLS/CPR certification per state requirements
• Preferred: 1+ year PT experience in neuro rehabilitation
• Demonstrates knowledge of rehabilitation techniques for complex neurological injuries
• Strong communication, both verbal and written

Responsibilities

• Perform evaluations and develop treatment plans for home and community settings
• Regularly reassess treatment effectiveness and participate in team conferences
• Conduct discharge evaluations and provide home safety recommendations
• Communicate patient needs and progress to the care team and families
• Collaborate with other disciplines to optimize patient outcomes
