Job summary

Qualifications

• Education/Experience: at least 30 semester units in behavioral or social science; or 60 units including 15 in behavioral science; or 1 year of direct mental health services.
• A bachelor’s degree in behavioral or social science may be listed in lieu of detailed coursework.
• A valid California driver's license is required.
• Must read, speak and write fluent English.
• No supervisory responsibilities.

Responsibilities

• Assist Facilitator in teaching family members to locate resources.
• Attend Child/Family Team meetings to set goals and plans.
• Provide support for family/child in meetings and system areas.
• Develop MOUs with agencies for services.
• Provide Intensive Home-Based Services (IHBS).
• Transport clients in a company vehicle to meetings and appointments.
• Maintain charting per DBH and regulatory standards.
• Assist with crisis/emergency services.
• Provide activities aligned with ISP goals and family needs.
• Promote collaboration among families, advocates, mental health providers, healthcare providers, school personnel, and other agencies.
• Perform other duties with Clinic Director approval.
• Collaborate with Clinician, Facilitator, Parent Partner, TBS, Coach, QA/Fiscal staff to improve outcomes.
• Attend county meetings as assigned.
• Participate in rotating On-Call Schedule including FURS.
• Offer In-Person services during evenings, weekends, and holidays.
• Utilize On-Call schedule to address regular and emergency needs.
• Use Electronic Health Record for timely documentation.
