Collage Home and Community Rehabilitation Partners is seeking an experienced Licensed Clinical Social Worker to provide patient-centered care in Anaheim, CA, delivering services in patients’ homes and communities. PRN opportunities available for clinicians who will see patients throughout their stay and participate in interdisciplinary treatment planning.

We offer an evidence-based approach, extensive collaboration with families and professionals, and treatment plans tailored to individual needs

Qualificações

• Master's degree in Social Work and state licensure as an LCSW required.
• Excellent planning, organization, writing, and time management skills required.
• Ability to communicate with families, professionals, and co-workers.

Responsabilidades

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans.
