Qualifications

• Good organization and communication skills with minimal supervision.

Responsibilities

• Provides direction, supervision and evaluation of medical social services in patients' homes.
• Instructs, treats, observes and evaluates patients with social and emotional needs.
• Assists physicians and health team to understand social factors affecting health.
• Participates in discharge planning and care coordination activities.
• Maintains documentation and participates in performance improvement activities.
