Job summary

Apploi is seeking a compassionate Nurse Practitioner to provide hospice care in the Brookings, Oregon area. You will evaluate patients, develop care plans, prescribe treatments under physician supervision, and coordinate with the hospice interdisciplinary team to meet patient needs.

This role emphasizes home-based patient visits, clinical assessments, and ensuring quality end-of-life care in accordance with state regulations.

Qualifications

Graduation from an accredited School of Nursing.
Has a current DEA license.
Completion of an accredited Nurse Practitioner Program as per Board requirements or equivalent.
Current nursing licensure in state and CPR certification.
Master's degree with at least 1 year Nurse Practitioner experience and at least 1 year hospice/palliative care experience.

Responsibilities

Provides medical care and manages terminal illness under physician supervision.
Completes history/physical exams and documents findings in medical records.
Develops individualized care plans and prescribes medications/treatments.
Assesses hospice eligibility and collaborates with hospice team for certifications.
Communicates with attending physician and interdisciplinary team regarding patient needs.
Educates patients/families on preventive care and end-of-life issues.
