Qualifications

• MSW required for position.
• Driver's license required.
• 2+ years of clinical experience in a medical setting.
• 1+ years’ experience with geriatric patients preferred.
• Clinical licensure (LCSW/LICSW/LMHC) preferred.

Responsibilities

• Screening and ongoing assessment of psychosocial needs.
• Develop and implement comprehensive treatment plans with GRACE team.
• Conduct in-home geriatric assessments with GRACE team.
• Provide education on community resources and benefits.
• Advocate for access to care and resources in the community.
• Collaborate with GRACE interdisciplinary team and primary care providers.
• Offer supportive counseling to patients and families.
• Coordinate care and referrals, monitor plan effectiveness.
• Document interactions in medical record accurately.
