Qualifications

Responsibilities

• Performs psychosocial assessments of patients and families.
• Formulates biopsychosocial assessment, disposition and treatment plans.
• Provides mandated safety assessments when abuse is suspected and coordinates discharge planning.
• Collaborates with the interdisciplinary team to evaluate medical needs and plan care.
• Monitors progress, documents records, and addresses social determinants of health.
