FOX Rehabilitation in McLean, VA is seeking licensed Physical Therapists to provide one-on-one home-based care to older adults, delivering mobility and independence improvements through the Geriatric House Calls model.

You will set your own schedule with autonomy and flexibility, collaborate with an interdisciplinary team, and may include telehealth to extend access and continuity of care.

Qualifications

• Licensed Physical Therapist in Virginia or eligible to obtain licensure.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilités

• Provide one-on-one PT to older adults in homes or senior living settings.
• Perform evaluations, create plans of care, and deliver evidence-based treatment.
• Deliver sessions to improve mobility, independence, and function.
• Document in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
