Job summary

Northeast Family Services is seeking an ISO Home Based Clinician to deliver in-home services to children, youth and families in New Hampshire. You will act as the primary therapist, coordinating with DCYF, schools and other providers to implement treatment plans and ensure quality service delivery.

The role requires a Master’s degree (or Bachelor’s with substantial experience) in social work, counseling, or related field, plus valid driver’s license and reliable transportation.

Qualifications

• Master’s Degree with a major in social work, counseling, psychology or related human services field valid under NH state guidelines.
• Two years minimum clinical experience working with children, adolescents, youth and families in a home or community setting preferred.
• Bachelor’s Degree with a major in social work, counseling, psychology or related human services field valid under NH state guidelines.

Responsibilities

• Maintains assigned caseload and ensures proper implementation of in-home services per referrals by DCYF.
• Collaborates with DCYF to respond to referrals, develop treatment plans, participate in meetings, and document regularly.
• Develops treatment plans with measurable goals in collaboration with families and DCYF.
