Job summary

Northeast Family Services in New Hampshire seeks an ISO Home Based Clinician to serve as a primary therapist for in-home clients, coordinating with DCYF and the clinical supervisor. You will develop treatment plans, document effectively, and provide family-focused therapeutic interventions in home and community settings.

Qualifications

• Master’s Degree in social work, counseling, psychology or related human services field per NH guidelines.
• Two years minimum clinical experience with children, adolescents, youth and families in home or community settings preferred.
• Bachelor’s Degree with a major in social work, counseling, psychology or related field per NH guidelines.
• Five years minimum clinical experience with children, adolescents, youth and families in home or community settings preferred.
• Valid driver’s license with reliable access to a vehicle; proof of insurance required.
• Applicants must be authorized to work in the U.S.; we do not sponsor visas.

Responsibilities

• Maintains assigned caseload and ensures proper in-home service delivery per referrals.
• Works with DCYF to respond to referrals, create treatment plans, attend meetings, and document.
• Develops treatment plans with measurable goals in collaboration with families and DCYF.
• Adheres to documentation requirements including assessment and report writing.
• Provide therapeutic interventions to children, adults, and families in home/community settings.
• Provide parent education and supervised visitation as needed.
• Serve as clinical lead on cases with multiple providers.
• Coordinate with DCYF, NH courts, schools, and other treatment team members.

Education
