Job summary

Northern Light Acadia Hospital in Belfast, ME seeks a full-time Behavioral Health case manager to support adults and children in the community and schools. You will work within a multidisciplinary team, coordinating mental health and medical care to improve outcomes.

Responsibilities include facilitating access to services, building therapeutic rapport, and documenting care plans in the EMR. A valid MHRT certification and a driver’s license are required.

Qualifications

• Certified as MHRT/C provisional or full certification, or eligible for certification in a reasonable time.
• A valid driver's license in good standing with access to a vehicle.

Responsibilities

• Coordinates and facilitates access to preventive and health promotion services, mental health and substance abuse disorder treatment, and care coordination services.
• Meet with clients via Zoom or face-to-face in their home or community.
• Build therapeutic rapport and adapt communication to fit each client's needs.
• Coordinate and facilitate team meetings with client care team.
• Maintain ongoing collateral contacts with supports and providers for the treatment plan.
• Identify client safety risks and communicate them to leadership.
• Document in the electronic medical record per policies (assessments, needs/resources, treatment plans).
• Participate in monthly supervision focused on education and case consultation.
