Job summary

Northern Light Acadia Hospital in Bangor, ME is seeking a Behavioral Health Case Manager to provide comprehensive case management within a multidisciplinary team. The role focuses on improving mental health and coordinating care with primary care providers for adults with serious mental illness and children with emotional disturbances.

The position offers a full-time schedule 8:00 AM to 4:30 PM, with a $2,500 sign-on bonus and a hybrid work option.

Qualifications

• MHRT/C provisional or full certification
• Valid driver's license with vehicle access
• Bachelor's degree or related field preferred

Responsibilities

• Coordinate access to preventive and health promotion services and treatment
• Meet clients face-to-face or via Zoom in homes or communities
• Build therapeutic rapport with diverse clients and adapt communication
• Lead and document in the electronic medical record per policy
• Participate in monthly supervision and case consultations
