Job summary

Summit Pointe is seeking an Assessment Clinician to conduct comprehensive bio-psychosocial assessments and develop person-centered plans for individuals with IDD, mental health, or medical challenges in the Battle Creek area. The role coordinates services, facilitates planning, and brokers care with diverse providers to ensure timely, goal-driven supports.

The ideal candidate holds a Master’s degree in psychology or social work with MI licensure or eligibility, plus 2+ years in care coordination

Qualifications

• Master’s degree in psychology, social work or related field with MI licensure or eligibility.
• Minimum 2 years of care coordination, case management, or direct service delivery.
• Knowledge of person-centered planning, IPOS development and regulations.
• Experience with EHR systems and trauma-informed care is preferred.
• Strong communication and organizational skills; ability to work independently and collaboratively.

Responsibilities

• Conduct initial, annual, and updated psychosocial assessments using standardized tools.
• Develop person-centered plans and coordinate with providers to achieve goals.
• Broker services, manage care transitions, and coordinate hospital discharge planning.
• Maintain knowledge of community resources and support referral pathways.
• Participate in trauma-informed system activities and regulatory compliance.
