Job summary

Hospice Hawai'i seeks a Nurse Case Manager to join a field-based care team across Oahu. You will collaborate with physicians and an Interdisciplinary Team to assess, plan, and evaluate care, ensuring medical necessity and compassionate nursing support.

The role requires a Hawai'i RN license, with BSN preferred and about 1 year of acute hospital experience. Travel and periodic on-call may be required to deliver high-quality patient outcomes.

Qualifications

• Valid Registered Nurse License in Hawai'i.
• BSN degree preferred.
• 1 year of experience as a staff nurse in an acute hospital; hospice/home health experience preferred.

Responsibilities

• Collaborates with physician and Interdisciplinary Team to assess, plan, implement, and evaluate the plan of care for patients and their caregivers.
• Utilizes palliative care and holistic philosophy to provide care.
• Provides direct supervision of Hospice Aides during care delivery.
• Offers case management services to support patient care needs and ensure alignment with care goals.
