Navian Hawaii is seeking a Nurse Case Manager to provide holistic palliative care and manage patient goals across Oahu. You will collaborate with physicians and an interdisciplinary team to develop, implement, and adjust care plans while supervising Hospice Aides in daily care delivery.

The role requires Hawaii RN licensure, with BSN preferred and at least 1 year of acute care experience. Travel to patient homes and facilities is expected, with on-call duties and compliance responsibilities.

Qualifications

  • Valid Registered Nurse license in Hawai'i.
  • BSN preferred.
  • 1 year of staff nurse experience in an acute hospital setting; hospice/home health preferred.

Responsibilities

  • Collaborates with physician and Interdisciplinary Team to assess, plan, implement, and evaluate the plan of care for patients and their caregivers.
  • Utilizes principles of palliative care and holistic philosophy to provide holistic care.
  • Provides direct supervision of Hospice Aides during care delivery.
  • Offers case management services to support patient care needs and align with care goals.
  • Documents nursing and case management activities per hospice policies and regulatory standards.
  • Keeps attending physician informed of changes in patient condition.
  • Travels as required for patient/family care throughout Oahu.
  • Participates in on-call rotations, including evenings, weekends, or holidays.
  • Supports quality improvement and compliance initiatives.

Under the supervision of the Patient Care Manager, the Nurse Case Manager serves our patients by utilizing principles of palliative care and holistic philosophy to provide care that reflects skilled nursing judgment, medical necessity, measurable patient caregiver, and reasonable goals.

Primary Responsibilities

  • Collaborates with the physician and Interdisciplinary Team to assess, plan, implement, and evaluate the plan of care for patients and their caregivers
  • Utilizes principles of palliative care and holistic philosophy to provide holistic care
  • Provides direct supervision of Hospice Aides during care delivery
  • Offers case management services to support patient care needs, ensuring alignment with care goals and treatment plans
  • Provides case management services to best benefit patients and caregivers
  • Care provided reflects compliance with federal and state regulations, along with Hospice Hawai'i's policies and procedures
  • Documents all nursing and case management activities in accordance with hospice policies, procedures, and regulatory standards
  • Keeps the attending physician informed of changes in patient condition
  • Functions as primary nurse within the framework of an Interdisciplinary Team
  • Requests support services from other Interdisciplinary Team members as appropriate
  • Demonstrates good judgment in seeking and accepting guidance; facilitates clear and honest communication with other team members
  • Attends and actively participates in Interdisciplinary Team meetings and conferences
  • Maintains assigned productivity standards and manages a patient caseload as directed
  • Participates in assigned on-call rotations, including evenings, weekends, or holidays
  • Provides end-of-day report to the RN on-call at the end of each workday
  • Travels as required for patient/family care
  • Supports and participates in Quality Improvement, Performance Improvement, and Compliance initiatives in accordance with direction from the clinical leadership team
  • Attends required in-service education and professional development activities
  • Performs duties as assigned/required by the Patient Care Manager or Director of Clinical Operations

Knowledge and Skills Requirements

  • Ability to exercise sound, independent judgment in planning and providing care
  • Flexible and organized in planning
  • Skilled in providing holistic nursing care to patients and their families
  • Ability to identify and provide for the psychosocial needs of caregivers in concert with the other members of the Interdisciplinary Team
  • Ability to set priorities, research, problem-solve, and make independent decisions with minimal supervision by using logic to identify key facts, explore alternatives, and propose quality solutions within the scope of the job
  • Must be able to multi-task with the ability to meet multiple deadlines and possess strong organizational, planning, and follow-through skills
  • Excellent written and verbal communication skills as well as the ability to communicate clearly, concisely and effectively with all levels of individuals

Experience and Education Requirements

  • Valid Registered Nurse License in the State of Hawai'i
  • Bachelor of Science in Nursing (BSN) preferred
  • 1 year of experience as a staff nurse in an acute hospital
    • Hospice and/or home health nursing experience preferred

Employment Requirements

  • A legally compliant background check

  • TB clearance
  • CPR & BLS Certification

Working Conditions

  • Generally, indoors in-patient homes or Hospice Hawai'i homes, nursing, and acute care facilities; may include outdoors in all types of weather

Work Location

This position is based in the field, requiring travel to patient homes, facilities, and offices throughout Oahu using a personal motor vehicle

Hours

  • Generally, Monday through Friday 8am-4:30pm
    • Position may require weekend, night, or holiday availability

Physical, Mental, and Communication Demands

  • Job requires frequent standing, walking, and sitting; ability to carry 25 pounds, push and pull 50 pounds, and occasional climbing, kneeling, and reaching
  • Must have capability for frequent speech in English for ordinary conversation and conversation on the telephone
  • Must have ability to hear clearly and good sight acuity for both near and far including good depth perception, color, and full field vision
  • Cognitive abilities must include ability to multitask projects and demands, handle stressful situations, and be a collaborative team member
  • Must demonstrate ability to think critically, effectively analyze problems and arrive at well-reasoned effective solutions, and then apply those solutions appropriately
  • Requires learning a number of policies and procedures
  • Must know how to follow safety instructions
  • Requires ability to write and do arithmetic
  • In addition to being able to fulfill the physical, mental, and communication demands listed above, must be able to follow safety instructions and demonstrate critical thinking
