Job summary

Navian Hawai‘i is seeking a Nurse Case Manager to deliver palliative care in patient homes and facilities across Oahu. You will apply holistic nursing judgment to meet measurable goals and ensure medical necessity and care plan alignment.

Under the supervision of the Patient Care Manager, you collaborate with physicians and the Interdisciplinary Team, document care activities, participate in on-call rotations, and travel as required.

Qualifications

• Ability to exercise sound, independent judgment in planning and providing care.
• Flexible and organized in planning.
• Holistic nursing care for patients and families.
• Identify and address psychosocial needs of caregivers with team.
• Ability to set priorities, research, problem-solve with minimal supervision.
• Capable of multi-tasking and meeting multiple deadlines.
• Excellent written and verbal communication with all levels.

Responsibilities

• Collaborates with physician and Interdisciplinary Team to assess and plan care.
• Provides holistic care and supervises Hospice Aides.
• Provides case management to support patient needs and align with goals.
• Documents activities per policies, informs attending physician of changes.
• Participates in Interdisciplinary Team meetings and on-call rotations.
• Travel to patient homes across Oahu using a personal vehicle.
• Supports quality improvement and compliance initiatives.
