Job summary

Qualifications

• One year acute care nursing experience.
• Graduate of an accredited school of nursing.
• Per license requirements.

Responsibilities

• Clinical Practice and Care Management.
• Performs psychosocial/physiologic assessments that integrates changing data.
• Formulates a plan of care based on trends of similar patients, including patient education and discharge planning.
• Participates in planning routine transitional health care needs.
• Responds to changes in patient's needs and provides holistic care using independent clinical judgment.
• Adapts educational programs to individual patients and families.
• Ensures efficiency in the delivery of care and prioritizes needs.
• Advocates for patient rights and resolves conflicts.
• Evaluates patient outcomes and revises the plan of care.
• Document all patient care activities and assist with documentation process.
• Communicate and collaborate with health care team, patients and families.
• Appropriately delegates patient care activities and participates in team assignments.
• Participates in unit teams and committees.
• Contributes to unit quality improvement initiatives.
• Acts as a clinical resource and mentor to health care team members.
