Qualifications

• Graduate of accredited school of nursing.
• Current Louisiana State license as RN and BLS certification.
• Proficient in English with strong verbal and written communication and computer skills.
• One-year clinical care nursing experience.
• Per license requirements.

Responsibilities

• Clinical practice and care management throughout the patient continuum.
• Performs psychosocial/physiologic assessments that integrate changing data.
• Formulates a plan of care based on trends and includes patient education and discharge planning.
• Participates in planning routine transitional health care needs and end-of-life options.
• Communicates and collaborates with the health care team to maximize patient outcomes.
