Qualifications

Responsibilities

• Performs psychosocial/physiologic assessments.
• Formulates a plan of care based on trends and patient education.
• Coordinates discharge planning and patient/family education.
• Advocates for patient rights and resolves conflicts.
• Documents all patient care activities per standards.
• Collaborates with the health care team to maximize outcomes.
• Participates in unit quality improvement initiatives.
• Mentors and supports other team members.
