Job summary

Cornerstone Family Healthcare seeks a WHIN Peer Navigator to join our team in Newburgh, NY. This part-time role (up to 14 hours per week) focuses on outreach, education, and referrals to HIV/HCV testing and treatment services.

The position requires evening and weekend availability and supports SDOH through workshops and community events across Rockland, Dutchess, Orange and Westchester counties. Training provided; HIPAA guidelines strictly followed.

Qualifications

• Experience in outreach and health education.
• Ability to work evenings and weekends.
• Knowledge of HIPAA and confidentiality.

Responsibilities

• Deliver peer-driven services to program participants in the counties of Rockland, Dutchess, Orange and Westchester.
• Coordinate outreach to community partners and maintain collaborations with service providers.
• Facilitate groups and events for health education and risk reduction.
• Provide referrals for HIV/STI/HCV testing and linkage to treatment and services.
• Enter data into AIRS and ensure accurate reporting.
