Qualifikationen

• Unrestricted Advanced Practice License in the state of practice.
• AAHIVM credential/certification required or preferred.
• Minimum 2 years’ experience as an Advanced Practice Provider.
• BLS certification required.
• DEA registration required.
• Experience in population health management preferred.
• Experience with psychosocially and medically complex patients preferred.
• Knowledge of NCQA PCMH model preferred.
• Ability to work within a multidisciplinary team.
• Strong EMR experience and reporting capabilities.
• Excellent interpersonal and communication skills.
• Adaptability in a fast-paced environment.
• Proficiency with Microsoft Office and related tools.

Aufgaben

• Provide outpatient care for people living with HIV and other chronic infectious diseases.
• Develop and implement diagnostic and treatment care plans.
• Collaborate with primary care physicians and specialists to manage patient panels.
• Work in a multidisciplinary team to coordinate care across services.
• Educate patients and families about their conditions and care options.
• Lead transitional care plans across hospital discharges and home care.
• Utilize evidence-based medicine aligned with PCMH model.
• Support timely follow-ups and acute episode management.
• Assist with EMR reporting and care coordination analytics.
