Job summary

AbsoluteCARE Medical Center & Pharmacy is seeking an Advanced Practice Provider to deliver outpatient HIV and chronic infectious disease care within a large integrated team. You will develop diagnostic and treatment plans, educate patients, and coordinate care with social work, pharmacy, and other specialists.

You will lead care plans across the continuum, including hospital transitions and home care, while practicing evidence-based medicine in a PCMH framework.

Qualifications

• Unrestricted Advanced Practice License in the state of practice.
• Credentialed by AAHIVM.
• Minimum 2 years’ experience as an Advanced Practice Provider.
• BLS certification.
• DEA registration.
• Experience in population health management preferred.
• Experience with psychosocially and medically complex patients preferred.
• Knowledge of NCQA PCMH model preferred.
• Ability to work in a multidisciplinary team.

Responsibilities

• Provide outpatient care for HIV and chronic infectious diseases.
• Develop diagnostic and treatment plans with the care team.
• Collaborate with social work, care management, and pharmacy.
• Implement care plans with patient and family input.
• Develop transitional care plans across care continuum.
• Educate patients on disease management and advanced directives.
• Practice evidence-based medicine per PCMH model.
• Manage and coordinate HIV/AIDS patient care activities.
