Job summary

HNL Lab Medicine is seeking a case management professional to collaborate in a multidisciplinary team, assess patient and family needs, and develop a documented service coordination plan. You will advocate for patients and coordinate referrals across the network, ensuring plans meet grantor requirements.

The role involves planning care, conducting assessments, monitoring outcomes, and reporting data to improve service quality within the community.

Qualifications

• Bachelor's Degree in Healthcare, Human Services, Education, or related field.
• 2 years case management experience or 2 years experience in a related field.
• Knowledge of Medicare, Medicaid, and other state or federal funded assistant programs.
• Cultural competency in working with a diverse population.
• Knowledge of the client's diagnosis, prognosis, care needs, as well as cost and barriers to goal achievement.
• Knowledge of community resources that can be accessed to assist patients in maintaining housing, basic needs, and overall health care.
• American Heart Association Basic Life Support - State of Pennsylvania within 90 Days.

Responsibilities

• Plans and identifies immediate, short-term, and long-term care needs for patients/clients and sets appropriate patient/client goals and time frames for achievement.
• Conducts assessments of continuing care needs through direct observation, data analysis, and interviews to determine the need for intervention.
• Establishes appropriate monitoring tools to measure outcomes related to client/patient interventions.
• Evaluates and measures patient/ client responses to planning, services, and intervention to ensure quality of case management services.
• Screens patients/clients for services and intervention appropriately as evidenced by documentation.
• Documents functional, situational, and clinical assessment within 24 hours of patient/client contact.
• Reports client/patient based data to client/patient based teams to improve quality of case management services.
