Job summary

HNL Lab Medicine in Allentown, PA seeks a dedicated case manager to assess patient and family needs and coordinate services. You will develop and monitor care plans, advocate for patients, and facilitate referrals across the Lehigh Valley network to improve outcomes.

You will document assessments promptly, work within grantor guidelines, and collaborate with a multidisciplinary team to identify and meet short- and long-term care goals for diverse patient populations.

Qualifications

• Bachelor’s degree in healthcare, human services, education or related field.
• 2 years case management experience or 2 years experience in a related field.
• Knowledge of Medicare, Medicaid, and other funded programs for eligible individuals.
• Cultural competency in working with a diverse population.
• Understanding of client diagnosis, prognosis, care needs and barriers to goals.
• Knowledge of community resources to support housing and basic needs.

Responsibilities

• Plans and identifies immediate, short-term and long-term care needs and sets goals.
• Conducts assessments through observation, data analysis and interviews.
• Establishes monitoring tools to measure outcomes of interventions.
• Evaluates patient responses to planning, services and interventions.
• Documents assessments within 24 hours of contact and reports to teams.
