Job summary

HNL Lab Medicine, headquartered in Allentown, PA, seeks a dedicated case management professional to support comprehensive care planning and service coordination across the network. You will assess patient and family needs, develop documented plans, and advocate for needed services within a multidisciplinary team.

The role emphasizes timely assessments, documentation within 24 hours of contact, and ongoing evaluation of outcomes to improve care quality across our Lehigh Valley facilities.

Qualifications

  • Bachelor’s Degree in Healthcare, Human Services, Education, or related field
  • 2 years Case management experience or 2 years Experience in a related field
  • Knowledge of Medicare, Medicaid, and other state or federal funded assistant programs for eligible individuals
  • Cultural competency in working with a diverse population
  • Knowledge and understanding of the client's diagnosis, prognosis, care needs, as well as cost and barriers to goal achievement
  • Knowledge and understanding of community resources that can be accessed to assist patients in maintaining housing, basic needs, and overall health care
  • American Heart Association Basic Life Support - State of Pennsylvania within 90 Days

Responsibilities

  • Plans and identifies immediate, short-term, and long-term care needs for patients/clients and sets appropriate patient/client goals and time frames for achievement in order to achieve desired outcomes.
  • Conducts assessments of continuing care needs through direct observation, data analysis, and interviews in order to determine the need for intervention.
  • Establishes appropriate monitoring tools to measure outcomes related to client/patient interventions.
  • Evaluates and measures patient/ client responses to planning, services, and intervention in order to ensure the quality of case management services.
  • Screens patients/clients for services and intervention appropriately as evidenced by documentation.
  • Documents functional, situational, and clinical assessment within 24 hours of patient/client contact.

Job description

Work Schedule: M-F 8:30-5 Summary Functions as a member of a multidisciplinary team and utilizes input from other team members to make decisions. Conducts psychosocial assessment of patient and family needs and formulates a documented service coordination plan. Acts as a patient advocate by negotiating for necessary services and facilitates coordination of referrals throughout the network and the community. Develops treatment plans in accordance with grantor requirements. Minimum Qualifications
  • Bachelor’s Degree Healthcare, Human Services, Education, or related field
  • 2 years Case management experience or 2 years Experience in a related field

Preferred Qualifications
  • Bachelor’s Degree Social work, psychology, sociology or similar field
  • 1 year Case management experience in a health care setting
  • Bilingual
Physical Demands
  • Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
