• Strong interpersonal and crisis intervention skills.

Responsibilities

• Provides case management services for individuals in the Case Management Program.
• Assesses, evaluates, and monitors cases for service planning and coordination.
• Develops treatment/service plans to address health, social, or environmental needs.
• Facilitates access to appropriate health and support services.
• Provides counseling and education to promote treatment adherence.
• Coordinates with health and social care professionals for client welfare.
• Liaises with community agencies to ensure continuity of care.
• Evaluates eligibility for emergency financial assistance.
• Offers advocacy, referrals, and linkage to community supports.
• Provides crisis intervention and individual/group counseling.
• Maintains accurate records and prepares reports per standards.
• Participates in public awareness and education programs.
