Formación

• Master of Social Work degree from a CSWE-accredited program.
• One year of full-time social work experience in a health care setting or equivalent.
• Ability to conduct psychosocial assessments and develop individualized case management plans.

Responsabilidades

• Provide HIV/AIDS intake and case management to adult patients in Madison Clinic.
• Conduct comprehensive assessments of psychosocial, financial, medical support and environmental needs.
• Develop individualized case management service plans to assist patients with medical, social, financial, legal and home care services.
