Job summary

Apicha Community Health Center is seeking a full-time Patient Navigator for our Manhattan location. Qualifications

  • Associate’s degree in human services or social work and 12+ months of patient navigation or similar field.
  • Certification as a peer worker by NYS AIDS Institute or CUCS, or 90 hours training equivalent.
  • Strong socio-cultural identification with the target population or relevant experience.
  • Fluency in one or more Asian languages preferred.
  • Excellent written and verbal communication in English; ability to coordinate with medical staff.

Responsibilities

  • Provide direct services to persons living with HIV/AIDS under supervision.
  • Assist with executing medical and support service plans, including appointments, education, reassessment of entitlements, transportation, and childcare.
  • Conduct field-based tasks: observe patient self-administration of ARVs, report ARV side effects, deliver services in home, community settings, or other convenient locations.
  • Maintain client records and documentation per Apicha’s standards.
  • Contribute to quality assurance and CQI tasks within the unit.
  • Represent the agency at venues to promote services.
  • Perform other duties as assigned.

Job description

Apicha Community Health Center aims to improve community health by providing access to comprehensive primary care, preventive health services, mental health, and supportive services. The center is dedicated to delivering high-quality, culturally competent services that enhance the quality of life and advocates for underserved and vulnerable populations.

Position Summary

We're looking for a committed, professional full-time Patient Navigator for our Manhattan location. The ideal candidate is personable, works well with a diverse population, communicates effectively with medical providers and support staff, and has good attention to detail. The Patient Navigator provides field-based case management services to patients enrolled in the HIV care coordination program and is the main point of contact for the patient. The Patient Navigator builds strong relationships with HIV-positive patients to help them connect and stay engaged in prevention and treatment services, removes barriers to care, and helps them navigate the health care system while promoting their health.

Job Responsibilities
  • Provide direct services to persons living with HIV/AIDS under the supervision of a Care Coordinator.
  • Carry out tasks needed to execute medical and support service plans, including accompanying patients to appointments when required, providing coaching, delivering monthly or weekly health education encounters, performing entitlements reassessment, and coordinating logistics for plan adherence reminders, transportation, and childcare arrangements.
  • Perform field-based tasks: observe and record patient self-administration of ARVs, assess for and report any ARV-related side effects, and deliver services in the home, CBO, work, or any other convenient location for people living with HIV/AIDS.
  • Maintain client-related records and other required documentation according to the protocols and standards of Apicha’s Department of Support Services.
  • Work as part of the unit on quality assurance and continuous quality improvement (CQI) tasks.
  • Represent the agency at various venues to promote agency services.
  • Other activities as assigned.
Qualifications
  • Associate’s degree in human services or social work and at least 12 months of experience in patient navigation or a similar field.
  • Consideration of certification as a peer worker by the NYS AIDS Institute Peer Certification, case management certification by the CUCS, or at least 90 hours of documented training equivalent to those required by the NYS Peer Worker.
  • Strong socio-cultural identification with the target population or experience working with the target population.
  • Fluency in one or more Asian languages preferred.
  • Excellent written and verbal communication skills in English required.
  • Ability to communicate well with medical providers and support staff.
  • Ability to work well with diverse populations.
  • Ability to handle multiple tasks in a stressful environment.
  • Ability to work within the organizational structure.
  • Ability to work flexible hours, including some evenings and weekends.
  • Ability to use common office software (Word, Excel, PowerPoint); familiarity with eSHARE preferred.
