Qualifications

• Understanding of gender identity and LGBTQIA+ issues.
• Proficient in Microsoft Office and EMR systems.

Responsibilities

• Conduct psychosocial assessments and eligibility assessments for Ryan White program clients.
• Develop individualized service plans and monitor progress toward goals.
• Close gaps in medical care and support adherence to treatments.
• Manage active caseload with focus on 60% billable time.
• Document contacts and referrals in EMR within required timelines.
• Provide information and referrals to partner agencies as needed.
• Assist with insurance navigation and patient assistance programs.
