Qualifications

• RN license: BSN in CA or LVN with 1 year case management experience.
• Knowledge of Ryan White Services preferred.
• Experience coordinating with physicians and care teams.
• Ability to educate and follow up with clients.

Responsibilities

• Meet with patients to complete Needs Assessments and develop Care Plans.
• Provide ongoing education on diagnoses and medications.
• Coordinate with clinicians to optimize treatment regimens.
• Conduct retention calls to minimize gaps in care.
• Assist with ACA enrollment and access to resources.
