Qualifications

• Minimum of 1 year experience in providing HIV prevention or care services to people living with HIV.
• Hospital nursing care of HIV-infected persons also applicable.
• High School/GED required.
• ASN, or BA in Social work, psychology or public health preferred.
• Current NJ-RN License preferred, NJDHSTS certified HIV counselor preferred.
• Division of HIV, STD, and TB Services (DHSTS) clinical navigator trainings (will be provided)preferred

Responsibilities

• Educate patients on HIV, treatment adherence, and risk reduction.
• Provide care coordination and link patients to services such as case management, MAT, behavioral health, and social services.
• Conduct partner services, including contact tracing and HIV testing.
• Perform needs assessments and address barriers to care (e.g., housing, transportation, mental health).
• Build supportive relationships to promote care engagement and retention.
• Conduct outreach in hospitals, community settings, and jails to connect patients with care.
• Act as an advocate and resource navigator for patients.
