Job summary

AIDS Healthcare Foundation is seeking a Non-Medical Care Manager to coordinate care, conduct initial assessments, and develop individualized care plans addressing psychosocial needs for clients in HIV care programs. You will connect clients to housing, food, and clothing resources, facilitate referrals and support groups, and perform ongoing retention calls to ensure continuous access to services.

Qualifications

• Experience coordinating care and connecting clients to services.
• Ability to advocate for clients and work across teams to remove barriers.
• Strong collaboration and engagement skills with providers and community programs.

Responsibilities

• Complete Initial Assessments and develop individualized Care Plans for psychosocial needs and goals.
• Address basic needs (housing, food, clothing) and coordinate referrals to relevant programs.
• Facilitate referrals and support groups to enhance access to resources and retention.
• Conduct ongoing retention calls to ensure continuity of care and align with Ryan White efforts.
• Verify client eligibility for ACA and enroll those who qualify when appropriate.
