BrightStar Care of Tyler & Longview is seeking a skilled, compassionate Physical Therapist to join our nurse-led in-home care team. This PRN position offers flexible scheduling and the chance to make a meaningful difference in patients' recovery.

You will perform in-home evaluations, develop individualized plans with measurable goals, and work with our RN-led team to support independence in the patient's home.

Qualifikationen

• Doctor or Master of Physical Therapy from an accredited program.
• Current, unrestricted Texas PT license or PT Compact privilege.
• Current CPR/BLS certification.
• Strong communication and independent work ability.
• Valid Texas driver's license and auto insurance.
• Ability to pass background check, drug screen, TB screening.
• One year PT experience preferred; home health/geriatric experience a plus.

• Provide therapeutic exercise, gait and balance training, transfer training, and fall-prevention guidance.

• Educate clients, families, and caregivers on home exercises, safe handling, and home safety.
• Complete accurate visit documentation in the ABS system within 24 hours.
• Report changes in condition to the RN case manager and physician promptly.
• Follow infection-control, HIPAA, and agency policies; participate in case conferences as needed.

BrightStar Care of Tyler & Longview is seeking a skilled, compassionate Physical Therapist to join our nurse-led care team. Whether you're looking to supplement your current schedule or work on a flexible PRN basis, you can set your availability and accept visits that fit your life.

The Opportunity

• Pay: $48-$55 per hour, based on experience

• Schedule: PRN-as-needed home visits that fit your availability

• Territory: Longview, Kilgore, Gladewater, Henderson, and Lake Cherokee

• Reports to: Director of Nursing

• Support: Collaborate with our RN-led team while working independently in clients' homes

What You'll Do

• Conduct in-home evaluations, re-evaluations, and discharge assessments.

• Provide therapeutic exercise, gait and balance training, transfer training, and fall-prevention guidance.

• Recommend equipment and adaptive devices, and teach clients how to use them safely.

• Educate clients, families, and caregivers on home exercises, safe handling, and home safety.

• Complete accurate visit documentation in BrightStar's ABS clinical system within 24 hours.

• Promptly report changes in condition to the RN case manager and physician.

• Follow infection-control, HIPAA, and agency policies, and participate in case conferences as needed.

What You'll Bring

• Doctor or Master of Physical Therapy from an accredited program

• Current, unrestricted Texas PT license or valid PT Compact privilege to practice in Texas

• Current CPR/BLS certification

• Strong communication skills and confidence working independently

• Valid Texas driver's license, reliable transportation, and current auto insurance

• Ability to pass a background check, drug screen, and TB screening

One year or more of PT experience is preferred. Home health or geriatric experience is a plus.

This role requires driving between client homes throughout Gregg and Rusk Counties, assisting with up to 50 lbs, and kneeling, bending, and standing for extended periods.
