Job summary

BrightStar Care of Tyler & Longview is seeking a skilled Physical Therapist to join our nurse-led in-home care team on a PRN basis. You will perform in-home evaluations, develop treatment plans, and guide clients toward independence in their own homes across Longview and surrounding areas.

The role offers flexible scheduling, competitive pay of $48-$55 per hour, and requires a Texas PT license, DPT, CPR/BLS, driving, and ability to work independently while collaborating with an RN-led team.

Qualifications

• Doctor of Physical Therapy (DPT) or Master of Physical Therapy from an accredited program.
• Current, unrestricted Texas PT license or valid PT Compact privilege to practice in Texas.
• Current CPR/BLS certification.
• Strong communication skills and confidence working independently.
• Valid Texas driver's license, reliable transportation, and current auto insurance.
• Ability to pass a background check, drug screen, and TB screening.

Responsibilities

• Conduct in-home evaluations, re-evaluations, and discharge assessments.
• Develop individualized treatment plans with measurable goals in coordination with the physician and Director of Nursing.
• Provide therapeutic exercise, gait and balance training, transfer training, and fall-prevention guidance.
• Recommend equipment and adaptive devices, and teach clients how to use them safely.
• Educate clients, families, and caregivers on home exercises, safe handling, and home safety.
• Complete accurate visit documentation in BrightStar's ABS clinical system within 24 hours.
• Promptly report changes in condition to the RN case manager and physician.
• Follow infection-control, HIPAA, and agency policies, and participate in case conferences as needed.
