Job summary

Shelterhouse is seeking a HIPP Case Manager in Cincinnati to support homeless individuals through housing-focused case management, stabilization services, and long-term community support. You will assess needs, develop service plans, arrange housing opportunities, and coordinate services to promote housing retention.

The role requires a Bachelor's degree or equivalent experience, a valid driver’s license, and ability to work flexible hours including weekends and holidays.

Qualifications

  • Bachelor’s Degree preferred; equivalent education and experience may be considered
  • Experience in case management, ISP/case plan development, or housing services preferred
  • Experience working with homelessness, housing-first models, behavioral health, or community-based programs preferred
  • Valid driver’s license with acceptable driving record required
  • Must maintain access to privately owned vehicle during work hours
  • Must be approved by Shelterhouse insurance provider for driving responsibilities
  • Must be able to work flexible hours including weekends and holidays as needed

Responsibilities

  • Complete client intakes, assessments, reassessments, and eligibility determinations in accordance with program requirements.
  • Develop, implement, and maintain Individual Service Plans (ISPs) that support client goals, housing stability, and self-sufficiency.
  • Maintain accurate and timely assessments, progress notes, case notes, service plans, and other required documentation within VESTA and applicable systems.
  • Connect clients to housing opportunities and supportive services, including behavioral health, substance use treatment, employment services, healthcare, transportation, and community resources.
  • Assist clients with obtaining benefits, identification documents, and other resources necessary to achieve housing stability.
  • Conduct regular office visits, client meetings, and community-based contacts to monitor progress and address barriers to housing stability.
  • Conduct home visits as needed to support housing retention and program compliance.
  • Maintain contact with clients as needed following housing placement to support successful transitions and housing retention.
  • Collaborate with landlords, housing providers, and community partners to address challenges that may impact housing placement or housing retention.
  • Maintain ongoing communication with community service providers to facilitate referrals and coordinated care.

This role works directly with clients to assess needs, develop individualized service plans, connect individuals to housing opportunities, and coordinate supportive services that promote long-term housing retention and stability. The HIPP Case Manager plays a critical role in helping clients transition from homelessness into stable housing while supporting recovery, independence, and overall wellbeing through compassionate and client-centered case management.
