Job summary

Dartmouth-Hitchcock Health seeks a detail-oriented Health Information professional to organize, maintain, and disclose patient records in accordance with HIPAA, state and federal regulations. This role focuses on Release of Information and ensures timely, secure handling of sensitive data.

Responsibilities include reviewing requests, validating authorizations, processing subpoenas, coordinating storage, and upholding data integrity across paper and electronic records to support patient care.

Qualifications

• High school diploma or equivalent required.
• One year experience in a healthcare setting.

Responsibilities

• Prepares records for release following state and federal regulations.
• Reviews health information requests and ensures retrieved information is released.
• Verifies that patient authorizations are completed and HIPAA compliant.
• Processes subpoenas, court orders and other requests for records; reviews by RM/MR Manager or VP of Risk Management.
• Coordinates health information retrieval as necessary.
• Completes all requests for records no later than 30 days from receipt.
• Uses ROI software to record data for storage, retrieval, and reporting.
• Maintains knowledge of current HIPAA and release of information regulations.
• Works with patients and staff to provide information to enhance patient care.
• Organizes and manages health information data using various applications and databases.
• Ensures information quality, accessibility, and security in paper and electronic systems.
• Interacts with other healthcare practitioners to maintain complete health records.
• Applies information governance to health data and records.
• Responds to calls and directs to appropriate staff.
• Coordinates retrieval of health information from EMR and offsite storage.
• Assists with storage processes and performance improvement initiatives.
