Job summary

Astera Health in Wadena, MN is seeking a Health Information Management (HIM) Assistant to ensure accurate, secure handling of patient records. Responsibilities include assembling, scanning, indexing, and reviewing documentation in the EHR, and supporting release-of-information while protecting confidentiality.

You will assist with birth and death certificate data entry to Minnesota, maintain paper charts, and help with general HIM operations.

Qualifications

• One or more years of post-high school education in medical records and information systems.
• High school diploma or GED with relevant medical records experience.
• Strong written and oral communication; ability to organize and prioritize work.
• Teamwork, problem-solving, and good judgment; proficient in software/databases; Epic EHR preferred.

Responsibilities

• Assemble, scan and import patient documents into the EHR with quality review.
• Verify accuracy and completeness of medical records during deficiency analysis.
• Retrieve and deliver historical records for authorized personnel.
• Assist with release-of-information while protecting confidentiality and HIPAA.
• Support daily HIM operations and administrative tasks.
• Perform Birth and Death Certificate data entry for Minnesota.
• Maintain supplies, file charts, perform chart merges in the EHR.
• Ensure compliance with Joint Commission, CMS and state regulations.
