Job summary

TriHealth's High-Risk OB and NICU Clinic seeks a MSW to assess psychosocial needs of patients and families, develop and implement comprehensive discharge plans, and support complex readmissions. You will be a resource on social work, collaborating with the multidisciplinary team to ensure safe transitions and high-quality patient care.

This role emphasizes education, guidance for staff, and participation in specialty areas required by regulatory or grant funding.

Qualifications

• Master's Degree in Social Science or Social Work is required.
• Up to 1 year clinical healthcare or community social work experience.
• Social worker must be licensed.
• Grief and bereavement counseling, alcohol and drug intervention, and support for abuse/violence victims.

Responsibilities

• Applies root cause analysis to readmissions and collaborates with the health care team, patient, and family to resolve issues.
• Communicates with the multidisciplinary team to advise the plan and patient response; counsels caregivers on psychosocial interventions.
• Develops and implements care plans based on assessment, including counseling or crisis intervention; links to community resources.
• Promotes the profession of social work through education and rounds; serves as a resource and preceptor for new staff.
• Provides on-call services according to departmental policy with timely response and follow-up.
• Responds to referrals and assesses psychosocial and complex discharge needs using data from medical records and team input.
