Southwestern Behavioral Healthcare in Indiana is seeking a High Fidelity Wraparound Facilitator to oversee wraparound referrals and manage clinical care for youth and families within the Child & Family Services Division.

You will maintain credentialing, coordinate with partners, and ensure billing and documentation compliance while engaging in ongoing professional development.

Qualifications

• Must hold a degree and have hands-on clinical experience with youth.
• Qualifies as an OBHP (405 IAC 5-21.5-1(d)).
• Wrap Facilitator certification preferred; pursue DMHA/OMPP CMHW trainings.
• Eligibility for CANS SuperUser certification required.

Responsibilities

• Provide ongoing clinical management for all High Fidelity Wraparound referrals/cases.
• Maintain WF credentialing as required by Indiana and follow the wrap model.
• Coordinate with Southwestern staff and outside agencies for client care.
• Ensure compliance with Southwestern policies, including billing/docs.
• Commit to annual professional development opportunities.
• Collect and report data as required; use data to improve quality.
• Assess and respond to psychiatric emergent situations.
• Maintain documents and submit reports within time frames.
• Attend and participate in regular staff meetings.
• Facilitate child and family team meetings per the model.
• Participate in ongoing training to enhance performance.

Provide on-going clinical management for all High Fidelity Wraparound referrals/cases.2. Maintain WF credentialing as required by the state of Indiana and comply with the High Fidelity Wrap model currently being endorsed by the state.3. Coordinate with necessary Southwestern Behavioral staff and other agencies/organizations on behalf of coordinating care and services for clients and their families who are engaged in the Wraparound Facilitation program.4. Maintain contact with other service providers, community agencies, schools and civic organizations to enhance community awareness and public relations.MINIMUM QUALIFICATIONS:Bachelor’s degree in Social Work, Psychology, Sociology, Criminal Justice or other related degree with two or more years of clinical experience with youth.Qualifies as an Other Behavioral Health Professional (OBHP), as defined in 405 IAC 5-21.5-1(d).Certification by the state of Indiana as a Wrap Facilitator is preferred.Minimum of five years of experience in working with Seriously Emotionally Disturbed (SED) children and families preferred.Prior Wrap Facilitation experience preferred.Successful completion of DMHA and OMPP-approved training and certifications for CMHW services is required.Eligibility for certification as a CANS SuperUser is necessary.Should have obtained or be in the process of obtaining Wraparound Practitioner Certification in accordance with DMHA policy.Strong leadership, communication, and organizational skills are essential.A valid driver's license is required.Must obtain and maintain membership in the Southwestern Clinical Staff Organization (CSO).NAPPI (Non-Abusive Physical and Psychological Intervention) certification must be obtained and maintained.Candidates must pass required background checks including county/state checks, fingerprinting, CPS check, sex offender registry check and drug screen.If you are interested in joining a fun, friendly, innovative team, apply today!EOE/AA including Veterans and Disabled.If you are a person with a disability needing assistance with the application process, please call [phone].
